Answers to some of our patients most common questions.

Are you currently accepting new patients?

Yes, and we can often accomodate same day appointments.

What insurances do you accept?

We take most major insurances such as Blue Cross, Blue Sheild, Aetna, Riverside, Pacific Source and United Health Care. Currently we are not accepting new Medicaid, Medicare and some other non-major insurances. Please check with your individual insurance company to verify Davis Family Medicine’s participation with your insurance plan.

What should I bring to my first appointment?

Please bring your insurance card, photo ID, completed new patient forms, names and phone numbers of any former physicians (for prior records) and your co-payment. to change this text.

For subsequent appointments, please bring your insurance card and your co-payment and inform us of any address, phone or name changes.

How do I get a refill for my prescription?

For chronic illness, we provide enough refills to last until your next maintenance visit. If you have run out of refills, please schedule a new appointment!

Can someone explain my lab results to me?

It is our firm professional belief that to provide you with the best possible health care, we must meet with you face-to-face to review lab results, both normal and abnormal- an appointment must be scheduled to review lab results. We cannot review a lab report over the phone.

What if I am in a car accident?

Your health insurance will not cover an auto accident visit, so we cannot bill your health insurance if you come see us for an auto accident. We require that you pay for your visit at the time of the visit. We will give you everything you need to get reimbursed from your auto insurance for the visit.
